Below is the form to fill out for any dogs that are on the Charlie Boy Pet Wellness CBD program. Once you fill out and submit the form below it will email you a PDF that can be printed and posted with the dog.

This program is intended for dogs that are “challenging” to be adopted. This would include:  Older Dogs, Wounded/Recovering Dogs, Anxious Dogs, Hyperactive Dogs, Paired Dogs, and more.

This program is intended to help dogs who may “linger” in a shelter for longer as they require more attention or care post adoption. We receive a copy of ever form that is submitted and will post these PDF’s to our site as well.
